Click Here for Detailed Location Map Medical Records: For 6 months, begining February 17,2012, PFP will provide a digital copy of your medical records at no charge. the records will be copied on to a CD and will be mailed 15 business days after the date of receipt of request. If you would like a copy of your recrods, after the free grace period, please be aware that in accordance with the State of Texas Administrative Code there is a resonable fee of $25 for the first twenty pages and $.50 per page thereafter. The requested copies of medical and/or billing records shall be furnished within 15 business days after the date of receipt of the request and resonable fees for furnishing the information.
